Most urologists I speak with want to put me to sleep for adult circumcision. This brings added cost to having an adult circumcision. Is this really necessary? Bruce
99% of our adult circumcision patients are done under local anesthesia. We can give you IV conscious sedation if you wish. This means you will be awake, but like you had 3 martinis.
The medications we use are midazolam (Versed) and Fentanyl (a narcotic). If you receive sedation, you are medically-legally “intoxicated for 24 hours, so you will not be able to drive. Our approach is to apply a numbing cream to your penis for about 15 minutes. Then we inject local anesthesia mainly at the base, a “ring block.” We wait an additional 10 minutes or so for it to work. We pinch test you before we start to make sure you are thoroughly numb. If you feel anything we have plenty more anesthesia to give you, but most likely you’ll be fine. The anesthesia is long acting, lasts about 4 hours.
Anticipate you’ll be on monitors when you are in the operating room and later on, in the recovery room. You’ll be advised to stay in bed in our recovery room for a good 1 1/2 hours. You’ll be checked periodically to be sure you are doing well.
A double layered dressing is applied to minimize bruising and swelling. If you are from out of town, we ask that you stay at a very nearby hotel and continue essentially bed rest for the rest of the day. Then we’ll see you the next morning for a check up.
