Gynecomastia (male breast enlargement) or male boobs

Usually this is seen bilaterally, if on one side there is more reason to be concerned about a tumor.  In our practice the most common reason is associated with excessive weight gain.  Estrogens are produced in fat stores (even in men).  This enhances breast growth.  The issue is to determine how much is fat (in the breast) versus glandular tissue.  As a rule, fat will respond to liposuction, but often a central core of breast adenoma is also present which needs to be resected via a limited open incision , usually on the interior border of the areola (pigmented shield which surrounds the nipple.

Gynecomastia or enlargement of the male breast may produce an unwanted feminizing appearance. This is typically caused by an increase in stromal (connective) and ductal tissue, however a serious underlying cause should always be considered. The incidence of occurrence in naval military exams is about 8 in 100,000. Gynecomastia is noted normally to some degree in pubescence with incidence as high as 65% in boys aged 14 to 15 years. By late teens this finding lessens to 7.7%. Cosmetic surgeons consider 3 grades:

  1. a button of extra tissue under the areola (the pigmented area surrounding the nipple)
  2. diffuse enlargement with indistinct peripheral margins.

III. further mass effect with excessive skin involvement.

Many cases can be treated with liposuction techniques. Generally speaking only large cases with excessive non-elastic skin, or dense fibrous growth require open excision.
