I think general anesthesia is not required for an adult circumcision unless the patient specially specifies that, in which case we can bring in an anesthetist. Our technique is to offer if the patient requires a Xanax tablet early in the morning if the patient is especially anxious. After a thorough discussion of circumcision style preferences, a complete history and physical exam, the patient is then brought to the operating room and EMLA cream is applied for 15 minutes. This provides relative comfort so the patient doers not feel too much any needle stick when local anesthesia is injected, mainly at the base of the penis (a ring block). After 10 minutes the anesthetic is usually so effective no further local is required.
