Dear Stewart,

Your raphe, the pigmented line which starts in the scrotum and extends up the underside of the penile shaft should of course remain at 6 o’clock or be ventral.  When we begin to approximate penile skin, the first stitch is 6 o’clock to 6 o’clock.  The second stitch is at 12 o’clock and we continue bisecting.  Only if there is a waddle on the underside , the distal raphe is removed.  Please see before and after pictures of adult circumcision on our web-site adult-male-circumcision.com
